Overview: What Is the MRCS Exam?
The MRCS (Membership of the Royal Colleges of Surgeons) is a crucial postgraduate qualification required to enter higher surgical training (ST3+) in the UK and Ireland. It’s an intercollegiate exam—meaning one unified exam structure across four Royal Colleges:
- Royal College of Surgeons of England
- Royal College of Surgeons of Edinburgh
- Royal College of Physicians and Surgeons of Glasgow
- Royal College of Surgeons in Ireland
Structure: Exam Parts & Format
Part A – Written Examination (MCQ)
- Format: Five-hour computer-based test comprising two papers on the same day:
- Paper 1 (AM): Applied Basic Sciences — ~3 hours (~180 questions, two 90-minute segments separated by a short break)
- Paper 2 (PM): Principles of Surgery in General — ~2 hours (~120 questions)
- Attempts Allowed: Up to six attempts.
- Content: The exam tests a broad syllabus including:
- Basic surgical science and anatomy
- Surgical skills, patient assessment, perioperative care, trauma, pediatric surgery, transplant, professionalism, and leadership.
- Locations: Available via Pearson VUE test centres across the UK and internationally—including Pakistan
Part B – Clinical Qualification (OSCE)
- Format: An Objective Structured Clinical Examination (~3.5 hours) with 17 stations, each lasting ~9 minutes.
- Some stations assessed by two examiners, others by one

- Knowledge Stations: Anatomy, pathology, applied surgical science, critical care
- Skills Stations: Communication, history-taking, explanation to patients and professionals, clinical and procedural skills.

- Held three times annually—usually in January, April, and September:

Part B Fees and Dates

- £1,099 in Glasgow or London (Feb window)
- £1,132 for May window.
How to Apply
- Choose Your College: Apply via any of the four Royal College websites.
- Create an Account & Submit Application: Complete the online form and pay the fee.
- Supporting Documents: Upload or mail proof (e.g., medical degree, identity, registration) as specified. A signed declaration may be required .
- Scheduling: Choose a Pearson VUE centre when booking opens — typically about two months before exam date.
- Withdrawal & Refunds: If you withdraw before the closing date, you’ll get an 80% refund (20% administrative fee). After closing date, valid evidence must be submitted to qualify for any refund.
